Fasting is NOT obligatory upon the sick person who is not able to fast because of sickness or medical advice. But he will restitute what he missed when he is cured, even after many years. He is not supposed to feed the needy.
But if a person suffers from a disease which trustworthy medical expert confirm that it is difficult to cure, then he will not restitute missed fasting. Instead, he should feed the needy in lieu of each day of fasting missed.
The Wayfarer is also required to restitute what he missed as a result of his journey.
Evidence is in Surah Baqarah verse 184: "....And whoever is sick or on a journey then the same number should be made up from other days. And for those who find it hard to bear, there is a ransom by feeding a poor man..."
